You need your DigiD to make an appointment. You cannot use a DigiD of somebody else. Via this portal you can only make an online appointment for yourself.

First, we're going to ask you a number of questions. Then you can make your appointment. The questions you get will be about your health. For example, you may not have coronavirus symptoms such as a cold, a fever or tightness in your chest. Or you might be in a (medical) situation that is more important. After the questions, you will be able to choose from a number of dates and locations.
